University Parallel Dual Enrollment Advisement Appointment

When you attend your advisement appointment, you must bring:

-a SIGNED parental consent form and Web Application Signature Page (Your guidance counselor has these forms!)

-a copy of your {driver’s license} OR  a copy of your {driver’s permit AND a copy of your birth certificate}. If you do not have a Driver's License or Driver's Permit you will need to bring a copy of your Birth Certificate

-a copy of your ACT scores (If you have not taken the ACT, please let us know when you are scheduled to take the ACT at your appointment.  If you have previously taken the ACT, these scores can be printed from the ACT student portal.)

- High school Transcript

***IF YOU ARE A RETURNING DUAL ENROLLMENT STUDENT***:

-You will ALSO need to bring a copy of your Transcript if you attended another Institution other than Northeast State
